Job summary

Behavioral Health Solutions of South Texas is seeking a Community Health Worker in Pharr, TX to empower individuals and families to access behavioral health services and community resources. You will engage with trauma-exposed populations, promote program services, and build local partnerships to improve access to care.

The role requires a High School Diploma or GED (or equivalent) and a Texas CHW Certification or the ability to obtain it within 6 months.

Qualifications

• High School Diploma or GED or equivalent experience.
• 2+ years of experience in social services or working with at-risk populations.
• Current Texas driver’s license, reliable transportation, and liability insurance.
• Bilingual in English and Spanish preferred.
• Current Texas CHW Certification or ability to obtain within 6 months of hire.

Responsibilities

• Conduct outreach, presentations, and networking to engage individuals with trauma or behavioral health risk factors and promote program services.
• Build and maintain partnerships with community organizations by developing MOUs, coordinating referrals, and removing access barriers.
• Connect participants to behavioral health, healthcare, and community resources via referrals, care navigation, and follow-up.
• Facilitate educational and support groups focused on health, wellness, and community resources.
• Maintain accurate documentation and ensure compliance with policies and grant requirements.

Job Description

• Conduct community outreach, presentations, and networking activities to engage individuals with a history of trauma, substance use, mental health concerns, or other behavioral health risk factors while promoting awareness of available program services.
• Build and maintain partnerships with community organizations and social service agencies by developing Memoranda of Understanding (MOUs), coordinating referrals, and identifying barriers to accessing care.
• Connect participants to behavioral health, healthcare, and community resources by providing referrals, care navigation, follow-up, and appropriate interventions based on individual needs.
• Facilitate educational and support groups focused on health, wellness, and community resources.
• Maintain accurate documentation, support implementation of evidence-based program curricula, and ensure compliance with organizational policies and grant requirements.

Requirements

• High School Diploma, GED, or experience equivalency.
• At least 2 years of experience working in social services, networking with social service agencies, or working with at risk populations in the community or schools.
• Current Texas driver’s license, reliable transportation, and liability insurance are required.
• Bilingual in English and Spanish preferred.
• Current Texas Community Health Worker (CHW) Certification or ability to obtain certification within 6 months of hire.
