Job summary

One Home Health LLC is seeking a Part-Time/PRN Home Health Aide (HHA/CNA) to join our home care team serving Lake County & Porter County, IN. You will provide in-home personal care and support to patients in their residences, following care plans and ensuring comfort and safety.

Qualified CNAs with home health experience who are compassionate, patient-focused, reliable, and good communicators are encouraged to apply. Flexible scheduling and competitive pay offered.

Qualifications

• CNA credential required or current certification
• Prior home health or hospice experience preferred
• Ability to provide compassionate, patient-centered care
• Excellent communication and reliability

Responsibilities

• Provide routine, individualized personal care in patients’ homes
• Assist with bathing, dressing, grooming, and mobility
• Monitor and report changes in patient condition to the care team
• Provide basic health-related tasks such as medication reminders and bandage changes (as permitted)
• Support patient comfort, safety, and daily living needs
• Educate family members on proper care techniques when appropriate
• Follow the established plan of care and agency guidelines
• Maintain patient confidentiality and a safe home environment
