St. Croix Health is seeking a part-time (0.6) clinical Pharmacist to join our team. The role will float to all Pharmacy locations: St.

Croix Falls, Frederic and Balsam Lake, ensuring accurate dispensing and high-quality patient care. Responsibilities include patient counseling, medication management, and collaboration with physicians and nurses to optimize therapy. Wisconsin licensure, BLS, and ACPE-accredited PharmD are required or in progress; immunization certification is preferred within 6

Qualifications

• Pharmacy degree from an ACPE-accredited program.
• Current Wisconsin Pharmacist license.
• BLS certification required.
• Immunization certification preferred within 6 months of hire.

Responsibilities

• Dispense medications according to legal, regulatory and organizational standards.
• Provide patient counseling on proper medication use, side effects and interactions.
• Review prescriptions and medications to ensure safe and effective therapy.
• Maintain accurate patient records and document medication therapy management.
