Job summary

Valor Healthcare in Cookeville, TN is seeking a passionate Dietitian to join our Community Based Outpatient Clinic team for a part-time role two days per week. You will educate patients on food choices, design diets, and lead nutrition education sessions under physician guidance.

This position supports VA-care delivery, collaborates with the PACT team and community partners, and offers a weekday schedule with three weeks PTO to start and comprehensive benefits.

Qualifications

  • Bachelor's degree in Nutrition (required).
  • Master’s degree preferred.
  • Two years' experience with adult patients preferred.
  • Credentialing with VA and current BLS/ACLS as specified.

Responsibilities

  • Educate patients on food choices to promote health.
  • Plan diets and menus for conditions treatable with diet therapy.
  • Conduct nutrition education classes.
  • Create nutrition education materials for staff and patients.
  • Monitor patient communications (phone/email).

Valor Healthcare is looking for a passionate Dietitian to join our team at the Community Based Outpatient Clinic (CBOC) in Cookeville, TN. As a Dietitian, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.


This position is two days a week.


Core Responsibilities


  • Educate patients on how to select food to maintain and promote health as well as to prevent illness and disease.

  • Plan appropriate diets and menus to meet the needs of individuals whose medical condition can be treated or controlled using diet therapy.

  • Conduct nutrition education classes.

  • Serve as a community resource to patients.

  • Create in house nutrition education materials for use in nutrition education of staff and patients.

  • Provide information and education to staff members using nutrition education materials, or in- service/meetings per request.

  • Monitor and respond to phone and e-mail messages promptly.

  • Inputs all patients’ information into VISTA/CPRS.

  • Completes any and all clinical reminders “due” at the time of each patient visit.

  • Assists in co-managed care collaboration with non-VA providers, and facilities.

  • Maintains confidentiality of veterans’ data and information.

  • Complies with all VA and company training requirements.

  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission(JC), Environment of Care (EOC) oversight, lab compliance and other related items.

  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.

  • Must provide excellent customer service to each veteran and his/her family, both in person and over the phone, as well as to fellow colleagues and clinic visitors.

  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.


Qualifications


  • Bachelor's degree, or higher, in Nutrition from an accredited school; Master’s degree preferred.

  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.

  • Two (2) years' experience with adult patients preferred. Specific requirements could vary based on individual VA contract.

  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).

  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract

  • Proficiency in written and spoken English.

  • Strong computer skills, including EMR experience

  • Energetic and optimistic demeanor

  • This is considered a safety sensitive position.


Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity
