Job summary

Allied Caregivers in New Hampshire is seeking a part-time Intake Specialist to onboard clients by assessing care needs and explaining services. You will interact with clients, families, and caregivers to start non-medical home care.

This role emphasizes confidentiality, compliance, and professional communication, with responsibilities including documentation, service coordination, and follow-up to ensure satisfaction.

Qualifications

• High school diploma or GED required.
• LNA or CNA experience preferred.
• Comfortable using computers, tablets, and other technology.
• Ability to communicate effectively in English (reading, writing, and speaking).
• At least 18 years of age with a valid driver's license and reliable transportation.

Responsibilities

• Client Intake & Assessment - Serve as the primary contact for new clients and families, conducting initial in-home assessments to understand care needs, preferences, and concerns.
• Intake Documentation & Service Coordination - Complete and maintain intake paperwork, service agreements, consents, and client records while coordinating with scheduling and care teams.
• Client Communication & Follow-Up - Educate clients about agency services, policies, and pricing; address questions and follow up after intake.
• Care Observation & Communication - Observe safety hazards and changes; report to administrative and care coordination staff.
• Confidentiality, Compliance & Professionalism - Maintain HIPAA compliance and confidentiality; stay current on care practices.
