Job summary

SAFY, in Greenville, South Carolina, seeks a Part-Time Case Manager I to provide in-home support to foster families and children. The role starts in office and may move to hybrid depending on performance, with a schedule under 25 hours weekly.

The Case Manager will travel daily, conduct assessments, coordinate with agencies, and ensure documentation within policy timelines. A strong background in social services and driving is required.

Qualifications

  • Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, Sociology, or Child & Family Studies is required.
  • 3 or more years of experience working with families and children, child welfare, or in-home therapies is required.
  • Valid driver license, acceptable driving record and auto insurance coverage to meet travel requirements; may require transporting youth if needed.
  • Current certification in First Aid/ CPR, and de-escalation training.

Responsibilities

  • Travel daily to provide in-home, community-based services to families and youth on your caseload.
  • Complete referral reviews and intake assignments; communicate service availability to all assigned clients.
  • Provide instruction, intervention and assistance as necessary through independent judgment and collaboration with a treatment team.
  • Coordinate with county agency and attend family sessions to promote successful clinical interventions with foster youth.
  • Coordinate and monitor service provisions for youth and families in the community and with school officials, courts, and other parties involved with the client.
  • Develop and ensure completion of appropriate assessments including but not limited to: Individualized treatment plans, psychosocial diagnostic assessments (PSDs), and mental health service plans according to established time frames involving all appropriate parties as required by policy and regulations.
  • Timely document outcome of home visits, support sessions, safety assessments, interaction with parties involved in youths’ care, and incident reports as required by policy, regulations and contract requirements. Documentation time is 72 hours within the visits.
  • Ensure foster parents, youth and case manager needs are met during periods of staff transition.
  • May require participation in rotating on-call schedule.
  • Ability to work nontraditional hours daily/weekly to fulfill position duties; may include some evenings and/or weekends.

Education

Salary Range: $21.00 To $27.00 Hourly

SAFY is seeking a Part-Time (under 25 hours weekly) Case Manager I for our Greenville division in South Carolina. As a Case Manager, you will be responsible for providing in-home support services for foster parents and children in the Greenville area.

This position will start in office then can eventually move to hybrid depending on performance.

What You’ll Do as a Case Manager
  • Travel daily to provide in-home, community-based services to families and youth on your caseload. All visits should be within 1.5 hours of home location on most occasions.
  • Complete referral reviews and intake assignments; communicate service availability to all assigned clients.
  • Provide instruction, intervention and assistance as necessary through the exercise of independent judgment and knowledge or as part of a treatment team to preserve the family unit.
  • Coordinate with county agency as appropriate and attend family sessions to promote successful clinical interventions with foster youth.
  • Coordinate and monitor service provisions for youth and families in the community and with school officials, courts, and other appropriate parties involved with the client. This may include but is not limited to: mental health, medical, educational, psychological, vocational and social services.
  • Develop and ensure completion of appropriate assessments including but not limited to: Individualized treatment plans, psychosocial diagnostic assessments (PSDs), and mental health service plans according to established time frames involving all appropriate parties as required by policy and regulations of the State of South Carolina and/or contract.
  • Timely document outcome of home visits, support sessions, safety assessments, interaction with parties involved in youths’ care, and incident reports as required by policy, regulations and contract requirements. Documentation time is 72 hours within the visits.
  • Ensure foster parents, youth and case manager needs are met during periods of staff transition.

  • Targeted hiring range - $21.00 to $27.00 per hour
    • Compensation is commensurate with transferrable experience, education, and licensure of candidate, along with internal equity and budgeted amount for the role.
