Job summary

StoryPoint Of Rockford seeks a Caregiver to provide compassionate, personalized assistance for residents in our community. Duties include bathing, dining support, wheelchair/walker escorts, toileting and light incontinence care, morning/bedtime routines, and assisting with daily tasks.

The role emphasizes timely attendance, adherence to 1440 care standards, and clear communication with Wellness Leaders and residents’ families.

Qualifications

• High School Diploma or GED preferred.
• Previous Healthcare, Geriatrics or experience working with the elderly population preferred.
• Certified Nurse Aide (CNA) or Medical Assistant (MA) Certification preferred.
• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
• Occasionally lift and/or move up to 25 pounds.

Responsibilities

• Actively supports our culture – our pillars and 1440.
• Completion of daily/weekly tasks as assigned by the Wellness Coordinator, and assisting others routinely with their assignments.
• Provide compassionate and competent care for residents: bathing, dining assistance, wheelchair/walker escorts, toileting and light incontinence care, morning and bedtime routines, light room upkeep, answering call signals, medication assistance.
• Completes and documents records including pertinent information entered into log book.
• Maintains knowledge of safety and emergency procedures and adheres to company policies.
• Attends staff meetings, in-service programs, safety courses and other required meetings.
