Job summary

Axil Health is seeking a Care Navigator to support our clinical pharmacists in delivering Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. This part-time, on-site role is based out of our Raleigh, NC location, requiring MA or LPN certification and a high school diploma.

You will enroll patients into RPM or CCM programs, prepare charts ahead of CCM encounters, and perform monthly CCM and daily RPM check-ins, while communicating urgent health concerns to clinicians and

Qualifications

• MA or LPN certification is required and documentation must be provided.
• High school diploma or GED required.
• Professional telephone communication skills are essential.
• Working knowledge of Excel, Allscripts or similar EHR platforms, and data entry.
• Strong time management and ability to juggle priorities.
• Clear verbal and written communication with a patient-centered approach.

Responsibilities

• Enroll eligible patients into RPM or CCM programs.
• Prepare patient charts ahead of CCM encounters.
• Make monthly check-in calls to CCM patients.
• Make daily compliance calls to RPM patients.
• Communicate urgent health concerns and requests to clinicians promptly.
• Protect patient confidentiality at all times and maintain courtesy and respect.
