Axil Health is seeking a part-time Care Navigator to support our clinical pharmacists across North Carolina clinics. Based on-site in Raleigh, you will help manage a shared patient panel and serve as a consistent point of contact between visits.
You must hold a national MA or LPN certification and be able to provide documentation. Responsibilities include enrolling patients into RPM/CCM, preparing charts, and making routine calls while safeguarding privacy.
The Care Navigator supports our clinical pharmacists in delivering Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. Working across multiple North Carolina clinics, the Care Navigator helps manage a shared patient panel of 500 or more, serving as a consistent point of contact for patients between visits and a reliable second set of hands for the clinical team.
This is a part-time, on-site position based out of our Raleigh, NC location. Candidates must currently hold a nationally recognized Medical Assistant (MA) or Licensed Practical Nurse (LPN) certification and be able to provide a copy upon hire.
