Our outpatient clinics serve over 26,500 patients, focusing on preventing overdose deaths and supporting long-term recovery.

Day in the Life of a Care Manager
  • Connect monthly with patients to assess needs and provide resources.
  • Maintain relationships with community partners for patient support.
  • Collaborate with a dedicated team to ensure excellent patient care.
  • Participate in community events to promote awareness and our mission.
Requirements
  • High School Diploma or GED at minimum.
  • Preferred experience in fast-paced environments, healthcare, addiction medicine, or community health.
  • Experience in care planning and compliant documentation is a plus.
  • Lived addiction experience is advantageous.
Hours, Schedule, and Travel

Work approximately 20-25 hours weekly, primarily at local clinics in York and Lancaster, with some community engagement or office hours. Schedule includes:

  • Monday: 2:00pm - 6:00pm (York)
  • Tuesday: 10:00am - 1:00pm (Lancaster)
  • Wednesday: 9:00am - 4:00pm (York), 1:00pm - 5:00pm (Lancaster)
  • Thursday: 9:00am - 12:00pm (York)
