Job summary

The ACTS Foundation is seeking a Part-Time Care Manager to support CalAIM services in Tulare County. The role combines home-based and field-based work, with potential transition to full-time after a 90-day probationary period.

Qualifications

• Bilingual in English and Spanish (preferred).
• Experience in social work, case management, or healthcare coordination.
• Experience supporting CalAIM Birth Equity populations with culturally responsive care.
• RN, MSW, LCSW or related background is a plus.
• Excellent organizational skills and attention to detail.
• Strong communication, advocacy, and interpersonal skills.
• Commitment to serving vulnerable populations and health equity.
• Ability to learn independently and adapt quickly.
• Valid California driver’s license and reliable transportation; travel to clients as needed.

Responsibilities

• Provide direct case management and care coordination for CalAIM participants.
• Assist clients in navigating medical, behavioral health, and community-support services.
• Conduct needs assessments, develop care plans, maintain records, and ensure CalAIM compliance.
• Collaborate with healthcare providers, community partners, and internal teams.
• Advocate for clients, promoting self-sufficiency, dignity, and access to resources.

Preferred Qualifications
