Job summary

ACTS Foundation in California is seeking a compassionate, organized Part-Time Care Manager to join our CalAIM team. You will coordinate medical, behavioral health, and community services for Tulare County participants, promoting equity and coordinated care.

After a 90‑day probation, the role may transition to full‑time based on performance. Preference given to bilingual English/Spanish applicants with social work or case management experience and the ability to travel to clients.

Qualifications

• Bilingual English/Spanish (preferred).
• Experience in social work, case management, or healthcare coordination.
• Experience with CalAIM Birth Equity focus and culturally responsive care.
• RN, MSW, or LCSW or related background is a plus.
• Strong organizational skills and attention to detail.
• Excellent communication and interpersonal abilities.
• Commitment to serving vulnerable populations and health equity.
• Ability to travel to clients; valid CA driver's license.

Responsibilities

• Provide direct case management and care coordination for CalAIM participants.
• Assist clients in navigating medical, behavioral health, and community services.
• Develop care plans, conduct needs assessments, maintain records, and ensure compliance with CalAIM standards.
• Collaborate with healthcare providers, partners, and internal teams.

The ACTS Foundation is seeking a compassionate, organized, and mission-driven Part-Time Care Manager to join our CalAIM team. In this role, you will help bring CalAIM’s vision of improved care, equity, and coordinated services to life for individuals in Tulare county.

Preferred Qualifications

• Bilingual in English and Spanish (Preferred)
• Experience in social work, case management, or healthcare coordination
• Experience supporting CalAIM Birth Equity population of focus and providing culturally responsive care to communities impacted by maternal and infant health disparities.
• RN, MSW, LCSW, or related background is a plus
• Excellent organizational skills and attention to detail
• Strong communication, advocacy, and interpersonal skills
• Commitment to serving vulnerable populations and promoting health equity
• Ability to learn independently and adapt quickly (autodidact mindset)
• Requires a valid California driver’s license, reliable transportation, and the ability to travel to clients as needed
