Overview

Christian Hospital and Northwest HealthCare are seeking an experienced Mobile Integrated Health (MIH) paramedic to provide comprehensive post-hospital release follow-up care, conduct home physical exams, and collaborate with physicians to ensure continuity of care. The role includes monitoring medication, dressing changes, vital signs, and connecting patients to community resources.

Preferred Qualifications

• Class (E) license preferred.
• 2 years of Paramedic experience required.
• Illinois paramedic license and IDPH MIH program completion.

Role Purpose

A Mobile Integrated Health (MIH) paramedic has a comprehensive medical knowledge base and is able to provide post-hospitalization care for a broad range of medical conditions. The MIH paramedic will also be able to provide advanced level medical care including post‑discharge evaluations, primary care in the home, and address social determinants of health.
