DispatchHealth is seeking a Paramedic II to deliver hospital-level acute care in patients' homes in the Phoenix area. The role combines clinical expertise, mobile technology, and teamwork to provide timely, patient-centered care outside traditional settings.

You will perform high-acuity in-home care, coordinate with the virtual team, and document in the EHR. This per diem opportunity offers flexibility, ongoing training, and a chance to contribute to an innovative hospital-at-home program.

Qualifications

• High School Diploma or GED required.
• Current BLS and ACLS Certifications required.
• Active licensure or certification to perform as a Paramedic within the state of employment per state requirement.
• Valid driver’s license and acceptable driving record per policies.

Responsibilities

• Drive DispatchHealth branded vehicle to meet patients at their place of need and load/unload supplies for daily activities.
• Don and doff PPE appropriately and maintain infection prevention strategies.
• In-Home Acute Clinical Care: Provide high-acuity care in the patient’s home setting for infections, heart failure, pneumonia, and post-surgical recovery.
• Clinical & Environmental Assessments: Conduct thorough physical exams and assess home safety and factors impacting recovery.
• Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team and report findings to remote team.
• Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts.
• Patient & Caregiver Education: Deliver education on disease management and medications.
• Documentation & Compliance: Document assessments and outcomes in the EHR in real time.
• Interdisciplinary Collaboration: Coordinate with physicians, case managers, social services, and partners to ensure continuity of care.
• Program Support & Leadership: Participate in research, support program development, and mentor new staff.

How You'll Make an Impact

The Paramedic II plays a pivotal role in delivering hospital-level acute care directly in patients’ homes as part of an innovative hospital-at-home care model. This role leverages clinical expertise, mobile technology, and interdisciplinary teamwork to ensure high-quality, patient-centered care outside traditional hospital walls. This role improves patient outcomes by working closely with interdisciplinary teams. The Paramedic II, functions with an expanded scope of practice under approved medical protocols and direct delegation by a hospital physician to provide timely patient-centered interventions.

What You'll Do

• Drive DispatchHealth branded vehicle to meet patients at their place of need and load/unload supplies for daily activities (averaging 6-8 visits per 10- or 12-hour shift as scheduled).

• Don and doff PPE appropriately and maintain strict infection prevention strategies.

• In-Home Acute Clinical Care: Provide high-acuity care in the patient’s home setting forconditions such as infections, heart failure, pneumonia, and post-surgicalrecovery—offering an alternative to inpatient hospitalization.

• Clinical & Environmental Assessments: Conduct thorough physical exams and assessthe home environment for safety and factors that may impact recovery or the plan of care.Identify and escal

• Care Coordination with Virtual Team: Serve as an on-site extension of the virtual careteam. Maintain real-time communication with the remote command center nurse andphysician to report findings, collaborate on care decisions, receive treatment orders, andensure all aspects of the care plan are executed safely and effectively.

• Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions based on hospital orders and standing order protocols.

• Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and when to seek help. Use both verbal coaching and written materials to reinforce understanding.

• Documentation & Compliance: Document all assessments, interventions, communications, and outcomes in the electronic health record (EHR) system(s) in real time. Ensure care is delivered according to program protocols and clinical standards

• Interdisciplinary Collaboration: Collaborate with physicians, advanced practice providers, case managers, social services, and community partners to ensure continuity of care. Act as a liaison between the patient and healthcare system to eliminate gaps in care or missed services.

• Program Support & Leadership (as assigned): Participate in research initiatives, support program development, and mentor new staff as directed by leadership. Represent the program in community outreach or quality improvement activities when needed.

What You Need

EDUCATION AND EXPERIENCE:

• High School Diploma or GED

• Current BLS and ACLS Certifications

• Active licensure or certification to perform as a Paramedic within the state of employment per state requirement.

• Valid driver’s license and acceptable driving record in accordance with DispatchHealth’s driving and insurability policies.

PREFERRED QUALIFICATIONS:

• Degree from an accredited college or university

• Previous trainer or instructor experience

• Previous Critical Care experience

• Previous Mobile Integrated Healthcare (MIH) experience

• Community Paramedic Certification

SKILLS/ABILITIES:

• Effective verbal, written, and interpersonal communication skills

• Demonstrated ability to provide effective coaching and leadership

• Demonstrated teaching and educational facilitation skills

• Ability to adhere to established standards for educational quality

• Familiarity with basic computer applications and functions

• Minimum two (2) years patient facing experience.

• Previous experience of comprehensive history gathering and documentation within EMR

• Previous experience with procedural skillssuch as vascular access, medication administration, wound care, etc.

• Ability to interact with patients and families and develop rapport and trust, ability to communicate and provide verbal professional clinical presentations/summaries.

• Function independently with supervision and delegation from the medical director.

• Work as part of a team toresponsiblydeliver care with flexibility and open-mindedness.

• Adhere to all medical protocols, guidelines and legal regulations.

While performing the duties of this job, team members are regularly required to sit, stand, walk, reach with hands and arms, and to talk and hear. Work Environment:

This is a field-based role requiring attendance in the market location before and after each shift to prepare for field visits and gather and restock supplies. Depending on the market territory, it may require travel to practice locations in other states.
