Job summary

Acadian Health in Jacksonville is seeking a Community Paramedic to deliver hospital-level acute care directly in patients’ homes as part of our hospital-at-home program.

You will conduct high-acuity home assessments, coordinate with a virtual care team, educate patients and caregivers, and document care in the EHR while collaborating with clinicians to ensure safe, effective care outside traditional hospital settings.

Qualifications

  • High School Diploma or GED is required.
  • CPR and advanced life support certifications (BLS/ACLS); PHTLS/PALS preferred.
  • Paramedic licensure in the state of employment is required.
  • Ability to obtain CP-C or IBSC CP-C certification within two years of hire.

Responsibilities

  • Provide high-acuity in-home clinical care for conditions such as infections, heart failure, pneumonia, and post-surgical recovery.
  • Perform comprehensive physical assessments and safety/environmental checks in the home.
  • Coordinate care with the virtual team and remote command center nurse/physician.
  • Respond to crises and rapid alerts with in-home interventions per orders.
  • Educate patients and caregivers on disease management and medications.
  • Document assessments and interventions in the EHR in real time.
  • Collaborate with physicians, case managers, and community partners to ensure continuity of care.
  • Support program development and mentor new staff as directed.

Acadian Health has an exciting opportunity for Community Paramedics to join our team. The Community Paramedic plays a pivotal role in delivering hospital-level acute care directly in patients’ homes as part of an innovative hospital-at-home care model. This role leverages clinical expertise, mobile technology, and interdisciplinary teamwork to ensure high-quality, patient-centered care outside traditional hospital walls.

About the Role

This role involves providing high-acuity care in the patient’s home setting, conducting assessments, coordinating care with a virtual team, and educating patients and caregivers.

Responsibilities

  • In-Home Acute Clinical Care: Provide high-acuity care in the patient’s home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery—offering an alternative to inpatient hospitalization.
  • Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may impact recovery or the plan of care. Identify and elevate any deviations that could compromise patient outcomes.
  • Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team. Maintain real-time communication with the remote command center nurse and physician to report findings, collaborate on care decisions, receive treatment orders, and ensure all aspects of the care plan are executed safely and effectively.
  • Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions based on hospital orders and standing order protocols.
  • Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and when to seek help. Use both verbal coaching and written materials to reinforce understanding.
  • Documentation & Compliance: Document all assessments, interventions, communications, and outcomes in the electronic health record (EHR) system in real time. Ensure care is delivered according to program protocols and clinical standards.
  • Interdisciplinary Collaboration: Collaborate with physicians, advanced practice providers, case managers, social services, and community partners to ensure continuity of care. Act as a liaison between the patient and healthcare system to eliminate gaps in care or missed services.
  • Program Support & Leadership (as assigned): Participate in research initiatives, support program development, and mentor new staff as directed by leadership. Represent the program in community outreach or quality improvement activities when needed.

Qualifications

  • High School Diploma or GED
  • Current BLS and ACLS Certifications; PHTLS and PALS Certifications preferred
  • Required licensure to perform as a Paramedic within the state of employment
  • Ability to obtain:
  • a. Community Paramedicine (IBSC CP-C) certification or
  • c. obtain CP-C licensure in the state of employment within two years of hire date.
  • Effective verbal, written, and interpersonal communication skills
  • Demonstrated ability to provide effective coaching and leadership
  • Demonstrated teaching and educational facilitation skills
  • Ability to adhere to established standards for educational quality
  • Familiarity with basic computer applications and functions
  • Team-oriented and able to work effectively and efficiently with others
  • Valid driver’s license and acceptable driving record in accordance with Acadian’s driving and insurability policies

Preferred Skills

  • Degree from an accredited college or university
  • Previous trainer or instructor experience
  • Previous Critical Care experience

Pay range and compensation package

Position Type: Full-time

Shift: 2 days on, 2 days off, 3 days on
