Job summary

Anpex Health is seeking a Nurse Practitioner or Physician Assistant to join the Palliative & Primary Care team in New Jersey. You will provide comprehensive primary and palliative care focused on symptom management, quality of life, and goal-concordant care across complex cases.

The role emphasizes advance care planning, patient and family education, and collaboration with the interdisciplinary team to ensure treatments align with patient goals and prognosis.

Qualifications

• Master’s or Doctorate in NP/PA program
• Experience in primary care, geriatrics or post-acute care preferred but not required
• Palliative care training and certification provided

Responsibilities

• Provide primary and palliative care services, focusing on symptom management and quality of life
• Lead goals-of-care discussions and advance care planning
• Collaborate with interdisciplinary team to align treatments with patient goals
• Manage patient transitions and education for patients and families
