Job summary

TeamHealth in Chattanooga, TN seeks a Palliative Care Advanced Practice Clinician (NP/PA) to join our post-acute care team on a full-time basis. You will round in skilled nursing facilities with strong support, autonomy, and earning potential.

You will provide high-quality palliative care to a diverse patient population, collaborate with a multidisciplinary team, and benefit from onboarding and leadership development opportunities within a growing post-acute program.

Qualifications

• TN license as NP/PA and DEA registration.
• Minimum of 2 years’ experience in palliative care.
• Excellent communication and teamwork skills.
• Ability to work in a post-acute care setting.

Responsibilities

• Perform comprehensive patient assessments, including medical histories and physical exams.
• Develop, implement, and adjust individualized care plans based on patient needs.
• Monitor progress and coordinate care to optimize outcomes.
• Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate, timely medical records in compliance with regulations.
