Job summary

Sterling Physician Recruiters, Llc is seeking a Palliative (Hospice) Medicine NP/PA for a part-time, 20-hour/week position in NW Ohio (near Toledo). The role supports patients across clinic, home, extended care facilities and telehealth, with a flexible first-shift schedule from 8am to 5pm.

Ideal candidates will be Board Certified with Palliative/Hospice experience and an OH medical license or eligibility. Compensation includes base salary up to $65k plus a productivity bonus, PTO, and robust

• Board Certification from a recognized medical board.
• Palliative Medicine/Hospice experience required or preferred.
• OH state medical license or eligibility to obtain one.

Responsibilities

• Assess and manage patient care needs in outpatient, home, facility, and telehealth settings.
• Collaborate with physicians, nurses, social workers and scheduling staff to coordinate care.
• Discuss goals of care, prognosis, and palliative care vs hospice options with patients and families.
• Coordinate care across clinic, home, and facility settings.
• Support 6-8 patients per day with a flexible first-shift schedule (8am–5pm).

Palliative (Hospice) Medicine NP/PA

Part-time position (20 hours)

Join an integrative non-profit integrated regional health system in NW Ohio (outside of Toledo). Suburban community offering a high quality of life, affordable housing, and good school districts.

Flexible first-shift schedule, (8am to 5pm). Support6-8 patients per day in an outpatient clinical setting, as well aspatient homes, extended care facilities, and telehealth.Service area includes Hancock, Wyandot, and Hardin counties and part of Allen County.Assess and manage patient care needs, including symptom management.Collaborate with physicians, nurses, social workers, and scheduling staff.Discuss goals of care, prognosis, and palliative care versus hospice options with patients and families.Coordinate care across clinic, home, and facility settings.

Requirements:

• Board Certified
• Palliative Medicine/Hospice experience
• OH medical state license, or eligible

Compensation:

• Guaranteed base salary to $65k
• Productivity bonus (average $5k)
• PTO (pro-rated), including sick time
