Job summary

FirstHealth of the Carolinas is seeking a Palliative Care RN to educate patients and families about disease processes and end-of-life conversations. The role includes home or facility visits to monitor needs and symptom changes, with communication to the care team to coordinate management.

Ideal candidates will have a BSN, NC license, and 2+ years in nursing (palliative care preferred). A valid NC driver’s license and CPR are required; CHPN certification is encouraged after experience.

Qualifications

  • Graduate of approved school of nursing with current North Carolina license or RN multi-state licensure privilege through the Nursing Licensure Compact.
  • Requires BSN; minimum of two years previous experience in nursing, including acute care, med-surgical or home care nursing.
  • Palliative Care or end-of-life experience preferred.

Responsibilities

  • Educate patients and families about their disease processes, including end-of-life conversations.
  • Provide home or facility visits to follow up on medication adjustments/reconciliation and symptom evaluation.
  • Maintain open communication with Palliative Care providers, PCPs, or other providers involved on the patient’s team.
  • Act as a nurse navigator as needed.

Position Description

Palliative Care RN who will provide disease‑specific education to patients/families to include end‑of‑life conversations. Visits will occur in the patient’s home or facility to monitor patient needs, follow‑up on symptoms following medication changes, offer navigational support, and provide communication to the IDT to include internal/external providers and care team members.

Will provide home or facility visits for patients to follow‑up on medication adjustments/reconciliation, and evaluation of symptoms. Will have open communication with Palliative Care providers, PCP's, or other providers involved on the patient’s team related to the patient’s condition and needs. Will act as a nurse navigator as needed.

Qualifications
  • Graduate of approved school of nursing with current North Carolina license or RN multi‑state licensure privilege through the Nursing Licensure Compact. Requires BSN; minimum of two years previous experience in nursing to include acute care, med‑surgical or home care nursing. Palliative Care or end‑of‑life experience preferred.
  • Possess valid North Carolina Driver’s License.
  • National Certification (CHPN) recommended after completion of at least two years of full‑time as a RN in palliative nursing practice.
  • CPR required.
