Job summary

Sacbar seeks a Board-certified Physician specializing in Palliative Care to provide outpatient and inpatient consultation services within the VA Central California Healthcare System's Hospice/Palliative Care program.

The incumbent will develop evidence-based care plans, collaborate with the interdisciplinary team, and help optimize patient quality of life, goals of care, and symptom management.

Qualifications

• MD/DO degree from an school approved for the year of study
• Current, full and unrestricted medical license in the U.S.
• Board certification in Palliative Care or eligible for board certification
• Active medical licensure without restrictions in the United States

Responsibilities

• Provide outpatient and inpatient consultation services for palliative care.
• Support Hospice/Palliative Care Consultation Program.
• Develop and implement written plans of care with patients and families.
• Collaborate with interdisciplinary care teams to optimize care quality and access.
