Position Summary

The Nurse Practitioner - PPV models Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation. Duties include providing palliative care to patients with chronic, complex, or life‑threatening conditions at multiple locations, delivering assessments, disease and treatment education, symptom management, and collaborating with primary care providers and specialists. The NP also complies with practice standards and works with the assigned Compassus Medical Director, adjusting schedule to meet agency needs.

Responsibilities
Hospice Care Responsibilities
  • Perform routine and emergency reassessments, including recertification visits for patients in the 3rd benefit period or beyond, admissions, and transfers.
  • Record observations, treatments, and other pertinent information.
  • Communicate findings and symptom‑alleviation measures with Medical Director, attending physician, DCS, and IDT.
  • Provide physical and emotional care to support patients and families during crises.
  • Prevent unwarranted hospitalizations by meeting patient and family needs in the home.
  • Complete and submit required paperwork within 24 hours of visit completion.
  • Educate patients and caregivers on care, disease processes, dying process, and symptom control.
  • Perform other duties as assigned, including safeguarding confidentiality.
Palliative Care Responsibilities
  • Conduct routine and urgent assessments, maintaining minimum availability of 4 days per month.
  • Triage consults by patient acuity and perform bedside evaluations with family education.
  • Order, perform, and interpret laboratory and radiology tests within scope of practice.
  • Prescribe medications, including controlled substances, per delegation and licensing.
  • Order treatments and durable medical equipment as indicated.
  • Assist in all facets of care coordination and referrals.
  • Prepare and maintain accurate patient records, charts, and documentation to support reimbursement.
  • Comply with applicable laws and regulations regarding collaborative agreements.
  • Complete accurate billing and coding for all patient encounters per CMS guidelines.
  • Define professional growth goals and participate in professional activities.
  • Engage in self‑care activities for personal and professional longevity.
  • Supervise nurse trainees and participate in supervision of other trainees.
  • Perform additional duties as assigned.
Interdisciplinary Team Collaboration
  • Collaborate and communicate with IDT to identify and address psycho‑social needs.
  • Notify IDT of patient and family needs and complete required documentation within 24 hours.
  • Educate patient and caregiver on disease process, goal setting, symptom control, and advance care planning.
Qualifications
Education and Experience
  • Master’s degree in Nursing required.
  • Minimum of one (1) year of nursing practice for hospice care; minimum of five (5) years of nursing experience for palliative care.
  • Experience in hospice, oncology, home health or long‑term care preferred for hospice; oncology, hospice, geriatrics, or intensive care preferred for palliative care.
Skills and Abilities
  • Mathematical skills: basic calculations in all units of measure.
  • Language skills: ability to read, analyze, and interpret professional journals and regulations; strong written and verbal communication.
  • Organizational and time‑management skills; ability to work flexible schedule, including nights and weekends.
  • Travel required.
  • Maintain confidentiality of patient and company information.
  • Strong organizational and time‑management skills.
  • Practice hospice philosophy.
Certifications, Licenses, and Registrations
  • Active, unencumbered CNS or Nurse Practitioner license in state of employment.
  • Nurse Practitioner certification in Adult, Family, Acute, or Geriatric required.
  • Active NPI number.
  • Maintain DEA certification if state allows prescription of controlled substances by APPs.
  • Board Certified in Hospice and Palliative Care preferred.
State Specific Requirements
  • Current collaborative physician agreement required in all states except Iowa.
Physical Demands and Work Environment
  • Regular standing, walking, and manual dexterity; lift and move objects up to 50 pounds.
  • Visual acuity requirements: close and distance vision, color and peripheral vision, depth perception.
  • Exposure to bodily fluids, infectious diseases, standard medical equipment, and tools.
  • Adaptations possible for disabilities.
