Job summary

Compassus is seeking a Nurse Practitioner for PPV to deliver palliative and hospice care, ensuring regulatory compliance while providing compassionate, patient-centered care.

The role covers routine and emergency assessments, management of acute and chronic conditions, order and interpretation of labs/imaging, and collaboration with the interdisciplinary team to optimize patient outcomes.

Qualifications

  • Master’s degree in Nursing or related field.
  • Minimum of one year of nursing practice for hospice care; or five years of nursing experience for palliative care.
  • Clinical nursing care experience in oncology, hospice, geriatrics, intensive care, or long‑term care preferred.
  • Strong written and verbal communication skills; ability to present information clearly to diverse audiences.
  • Excellent organization and time‑management skills with flexibility for nights, weekends, and travel.

Responsibilities

  • Document observations, treatments, and other pertinent information promptly.
  • Communicate findings and symptom management strategies to the medical director, attending physician, and interdisciplinary team.
  • Provide physical and emotional care to patients and families during crises, preventing unwarranted hospitalizations.
  • Manage acute and chronic health conditions through diagnosis, treatment, and monitoring.
  • Order, perform, and interpret laboratory and radiology tests within scope of professional practice.
  • Prescribe medications, including controlled substances, and order durable medical equipment as indicated.
  • Consult with palliative care physicians or designees; inform primary physicians of services provided.
  • Coordinate care referrals and collaborate with other health professionals to develop comprehensive care plans.
  • Maintain accurate patient records, charts, and documentation to support medical practice and reimbursement.
  • Provide patient and caregiver education on disease processes, symptom control, advanced care planning, and wound care.
  • Assist in the development and implementation of clinical practice guidelines and quality improvement initiatives.
  • Complete required paperwork and clinical documentation within 24 hours of visit completion.
  • Participate in interdisciplinary team meetings and monthly IDT discussions.

Skills & Abilities
  • Mathematical proficiency in basic arithmetic, rate, ratio, and percentage calculations.
  • Analytical reading and interpretation of professional journals, technical procedures, and regulatory documents.
  • Proficiency in English communication, both spoken and written.
  • Strong organizational skills and time‑management to balance clinical duties and documentation.
Certifications & Licensure
  • Active Nurse Practitioner license in the state of employment (Adult, Family, Acute, or Geriatric).
  • Active NPI number.
  • DEA certification retained if the state permits prescription of controlled substances by an advanced practice provider.
  • Board Certification in Hospice and Palliative Care is preferred but not required.
  • Current collaborative physician agreement in place (except Iowa).
Physical Demands & Work Environment

This role requires regular standing, walking, and manual dexterity, with the ability to lift and move objects up to 50 pounds. Visual acuity to close and distance, color, peripheral vision, depth perception, and focus adjustment is necessary. Exposure to bodily fluids and infectious diseases is inherent in a healthcare setting. Routine use of standard medical equipment and tools associated with clinical care is essential.
