Job summary

Bluegrass Care Navigators is seeking a Palliative Care Nurse to assess and address the needs of patients and families, coordinating with the IDG to ensure high-quality, compassionate care across home, facility, and hospital settings.

You will document care plans, participate in interdisciplinary rounds, and support patients with symptom management, advanced directives, and end-of-life care in alignment with the care plan and organizational policies.

Qualifications

• Registered nurse licensed in Kentucky or participating COMPACT state.
• At least 1 year of nursing experience; palliative care preferred.
• Strong assessment, documentation and communication skills.
• Ability to coordinate with IDG and manage multiple tasks.

Responsibilities

• Assess and address patient and family needs within the care plan.
