Hospice of the Bluegrass Inc. is seeking a Palliative Care Nurse to assess and address the needs of patients and families across home, facility, and hospital settings. This role emphasizes interdisciplinary collaboration and compassionate, patient-centered care.

You will plan, implement, evaluate, and document individualized palliative nursing care in alignment with the care plan and department policies. Flexibility to work Mon-Fri with a 40-hour schedule and strong CPR credentials preferred.

Qualifications

• Graduate of an accredited school for Registered Nurses.
• Currently licensed as an RN in Kentucky or COMPACT state.
• One year of nursing experience; palliative care preferred.
• Strong observation, judgment and communication skills.
• CPR certification current; ability to document accurately.

Responsabilités

• Assess and address the needs of patients and families in palliative care.
• Plan, implement, evaluate, and document individualized nursing care.
• Coordinate with IDG and other healthcare providers to ensure quality care.
• Care may occur in homes, SNFs, assisted living, or hospitals.
• Maintain professionalism and adhere to policy and standards.
