Job summary

Devoted Medical is seeking an experienced palliative care nurse to build and support a remote home-based care program. You will engage patients by video or phone, provide symptom management, and coordinate care across a multidisciplinary team.

The role requires ≥5 years RN experience including hospice/palliative care, BLS, and flexibility across time zones. Multistate licensure and strong tech skills are essential for success in a startup environment.

Qualifications

  • Unrestricted RN license, compact preferred.
  • Ability to get licensed in additional states within 90 days of hire.
  • Minimum 5 years RN experience, 2 years in hospice/palliative care; CHPN or willingness to obtain within 6 months.
  • Willingness to flex between time zones.
  • Comfort with technology and remote work.
  • Strong interpersonal/communication skills.
  • Active BLS certification.
  • Preferred: ELNEC/EPEC training.
  • Bilingual in English and Creole/Spanish/Vietnamese.
  • Prior ICU, oncology or geriatrics nursing experience.
  • Prior inpatient nursing experience.
  • Prior program development experience.

Responsibilities

  • Engage with patients telephonically or via video for home-based palliative care.
  • Coordinate care tasks, follow up on DME orders, work with home health staff, and communicate with PCP.
  • Participate in daily team huddles and weekly palliative rounds with the interdisciplinary team.
  • Maintain patient panels with routine check-ins and referrals as needed.
  • Collaborate with Devoted Medical teams to deliver integrated care.
  • Triage and respond to incoming calls from palliative patients.
  • Support palliative education initiatives for staff.
  • Work with teammates to grow the program and support patients.

Job Description A bit more about this role: Devoted Medical was founded on the belief that if we treat each patient like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. We help members navigate the healthcare system in a better and safer way. This means getting the right care at the right place at the right time. Our virtual Palliative Care program serves Devoted members with complex needs and life-limiting illness who otherwise would not have access to specialized palliative care. We pride ourselves on providing highly compassionate, patient-centered goals of care counseling, advanced care planning and symptom management for members projected to be in the final year of life. We meet patients where they are and support them in defining quality of life, achieving meaningful goals, and transitioning to hospice care. Virtual, multi-state palliative care is a novel concept and we are looking for a highly motivated, innovative, experienced palliative care nurse to help build the foundation of our clinical team. Our ideal clinical guide is caring, compassionate, solution-oriented and enthusiastic about providing an outstanding experience for Devoted Health’s members. They have excellent clinical judgment and triage skills. They are ready to innovate, adaptable to a continuously evolving startup environment, and willing to start scrappy, working with the whole Devoted family to create a revolution in healthcare.

Responsibilities
  • Engage with patients telephonically and/or via video to provide home-based palliative care rooted in compassionate, evidence-based clinical practice
  • Perform essential care coordination and case management tasks (e.g. following up on DME orders, collaborating with home health workers, communicating with PCP, etc.)
  • Participate in daily team huddles and weekly palliative rounds with interdisciplinary team
  • Maintain patient panel; conduct routine check-ins based on risk stratification or change in health status; refer to palliative social worker and NP as needed
  • Collaborate with other Devoted Medical teams as needed to provide the highest quality of integrated care for our shared patients
  • Assist with triaging and responding to incoming calls from palliative patients
  • Support palliative education initiatives for Devoted Medical employees in collaboration with palliative teammates
  • Work closely with your palliative care teammates to support, care for and grow the program, patients, and each other
  • This role will be require shift flexibility to cover member based in PST as needed
Required skills and experience
  • Unrestricted RN licensure, compact preferred.
  • Ability to get licensed in additional states if requested within 90 days of hire date
  • A minimum of 5 years of RN experience, with at least 2 years in hospice or palliative care CHPN® credential, or ability/ willingness to obtain within first six months of employment
  • Willingness to flex between time zones
  • Comfortable with technology and a fast-paced environment
  • Self-starter with ability to succeed in a work-from-home environment
  • Strong interpersonal and communication skills
  • Active Basic Life Support (BLS) certification
  • Preferred: ELNEC® or EPEC® training
  • Bilingual in English and Creole, Spanish, or Vietnamese
  • Prior ICU, oncology or geriatrics nursing experience
  • Prior inpatient nursing experience
  • Prior program development experience

Salary Range: $80,000 - $100,000 / year #LI-Remote The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
