Qualifications

• :Must hold a NP license with adult or family certification.
• :Hospice or palliative care experience preferred.
• :Strong communication and assessment skills.
• :Proficiency with EMR, documentation, and care coordination.

Responsibilities

• :Respond promptly to inpatient palliative care referrals and perform comprehensive assessments.
• :Develop and implement individualized care plans with an interdisciplinary team.
• :Prescribe medications within scope and conduct ongoing reassessments.
• :Lead or support goals‑of‑care conversations with patients and families.
• :Coordinate transitions to outpatient or hospice services and advise hospital staff.
• :Document assessments and plans in EMR in compliance with policies.
• :Engage in QA, utilization review, and performance improvement activities.
