The Role

Seeking 2 experienced Nurse Practitioners to support growing patient panels in Fargo. These are full-time positions with a mix of home visits and assisted living facility visits. Providers have autonomy in scheduling their patient visits and maintaining a balanced work-life rhythm.

Responsibilities

Provide high-quality primary and palliative care to geriatric patients in their homes and in assisted living facilities.
Maintain autonomy over scheduling to best serve patient needs.
Collaborate with medical team and participate in weekly team meetings.
Patient load: 4–6 per day on average

Qualifications

Current Nurse Practitioner license in North Dakota and Minnesota.
Minimum of 3 years NP experience; primary care with geriatric population strongly preferred.
Palliative or home-based care experience highly valued.
DEA registration required.
Ability to work independently in patient homes and manage a flexible patient load.
