Job description
Overview

We re hiring board-certified physician assistant/physician associate (PA) contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member's home. You will be provided a pre-populated, patient-specific form for each evaluation, and you do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Responsibilities
  • Conduct in-home health evaluations for eligible plan members (Medicare Advantage, Medicaid, and certain commercial plans).
  • Review medication and medical history and perform physical evaluation in the member's home using the provided form.
  • Follow-up, medication management, and additional treatment plans as directed by the onsite physician; participate in ongoing training and support provided by the physician.
Qualifications
  • Board-certified physician assistant (PA) or physician associate (PA) contractors eligible to perform in-home evaluations.
  • Ability to work as a contractor and manage in-home visits within assigned areas.
