Signify Health, part of CVS Health, aims to build trusted relationships to make people healthier. We are hiring board-certified nurse practitioner contractors to conduct in-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Evaluations include a medication and medical history review and a physical examination performed in the member's home.NP contractors can earn a competitive wage while managing other personal and clinical commitments.

Responsibilities

• Conduct in-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.
• Review medication and medical history and perform a physical evaluation in the home.
• Use a pre-populated, patient-specific form for each evaluation.
• Do not prescribe medicine, order lab tests, or alter the member’s current treatment regimen.

Qualifications

• Board-certified Nurse Practitioner (APRN).
• Experience in in-home or community-based care is preferred.
• Compassionate, with strong patient communication and the ability to work effectively in home settings.
