The South Bend Clinic Pain Management Eddy Street team is seeking a Nurse Practitioner or Physician Assistant to join a tight-knit, collaborative group delivering high-quality care close to home. Ideal candidate has 1-2 years of licensed AP provider experience in a clinical setting.

Qualifications

• Master's degree from an accredited PA or NP program.
• National certification in the applicable specialty (ANCC).
• Current state licensure as PA or Advanced Practice Nurse and RN.
• Maintains current CPR (BLS) certification.
• Active mid-level Practitioner Controlled Substance License and DEA License may be required based on prescriptive authority.

Responsibilities

• As an APP, perform patient examinations, develop care plans, and treat minor illnesses and injuries per protocols.
• Order labs, X-ray, and other non-invasive tests; refer to supervising physicians as needed.
• Prescribe medications within scope with physician oversight and licensing.
• Educate patients and families on health maintenance, prevention, and home care.
• Document progress accurately and maintain patient records.
