Job summary

Medley Health is seeking a compassionate Physician or Nurse Practitioner for a part-time pain management role across assisted living facilities. On-site work with strong administrative support and autonomy, managing pain through medications and procedures.

Responsibilities include evaluations, injections, and coordination with facility teams. NY license and DEA required for practitioners; board certification in pain management or related field preferred.

Qualifications

• MD/DO with pain management experience.
• Active New York medical license.
• Active DEA license required.
• Experience performing joint and/or trigger-point injections.
• Experience in assisted living, geriatrics or related settings preferred.

• Perform comprehensive pain management evaluations and follow-up visits in assisted living settings.
• Diagnose and manage acute and chronic pain conditions in older adults.
• Prescribe and manage medications, including controlled substances.
• Perform in-office interventional procedures such as joint injections and trigger-point injections.
• Develop individualized treatment plans focused on safe, effective pain control.
• Document assessments and progress notes accurately and timely.
• Communicate recommendations to facility clinical teams and assist with care transitions.

• NP: Competitive, based on experience and autonomy (discussed at interview)

About the Role

Medley Health is looking for a compassionate Physician or Nurse Practitioner for a part-time opportunity providing on-site pain management services across assisted living facilities. You'll work independently with strong administrative support, collaborating with interdisciplinary teams to manage residents' pain through medication management and interventional procedures.

What You'll Do

• Perform comprehensive pain management evaluations and follow-up visits in assisted living settings

• Diagnose and manage acute and chronic pain conditions in older adult populations

• Prescribe and manage medications, including controlled substances, in accordance with state and federal regulations

• Perform in-office interventional procedures such as joint injections and trigger-point injections

• Develop individualized treatment plans focused on safe, effective pain control

• Document assessments and progress notes accurately and timely

• Communicate recommendations to facility clinical teams and assist with care transitions as needed

Qualifications - Physician (MD/DO)

• MD or DO with experience in pain management; Board Certification/Eligibility in Pain Management, Anesthesiology, PM&R, or a related field preferred

• Active New York medical license

• Experience performing joint and/or trigger-point injections

• Experience in assisted living, geriatric care, or related settings preferred

• Strong clinical judgment, documentation, and communication skills

Qualifications - Nurse Practitioner

• NP license with pain management experience (Adult-Gerontology preferred; FNP considered with relevant experience)

• Active New York NP license

• Active DEA license required

• Experience performing joint and/or trigger-point injections

• Experience in assisted living, geriatrics, orthopedics, or internal medicine preferred

• Ability to work autonomously and manage a patient panel

• Strong documentation and communication skills
