Qualifications

• ACPE accreditation or FPGEC certificate requirements.
• Completion of clinical pharmacy internship, residency or fellowship.

Responsibilities

• Assess patients' response to drug therapy and plan therapy based on diagnoses.
• Order and evaluate labs to monitor drug therapy effects.
• Initiate, modify or discontinue medications for chronic disease management.
• Monitor pharmacotherapy and adjust dosing as needed.
