Job summary

Hampshire Hospitals NHS Foundation Trust is recruiting a paediatric nurse to join the Community Children’s Home Acute Team (CHAT) in Winchester. The role focuses on delivering acute nursing care at home to acutely unwell children, enabling early discharge and reducing hospital admissions.

You will work autonomously within a multidisciplinary community team, develop ambulatory care pathways, and be a role model for nurses and students.

Qualifications

• Experienced paediatric nurse with autonomous practice.
• Experience in acute and community settings preferred.
• Ability to work within CHAT and ambulatory pathways.

Responsibilities

• Care for acutely unwell children at home.
• Deliver acute nursing care in the community and ambulatory care.
• Develop ambulatory and unscheduled care pathways to prevent admissions.
• Facilitate early discharge from inpatient wards.
