Responsibilities

• Assess physical, social, and psychological status; gather information from patient, family, and team.
• Plan and develop individualized care plans with patient and family involvement.
• Implement care and coordinate with interdisciplinary team; manage time, supplies, and resources.
• Evaluate responses to interventions and revise plans as needed.
• Promote professionalism, ethical standards, documentation accuracy.
