As part of ThriveAP’s transition-to-practice, new graduate NPs and PAs receive mentored clinical education and ongoing professional development to accelerate readiness and build lasting patient relationships across diverse populations in Centra’s not-for-profit health system.

Job description

Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living.
PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.
