Job summary

Mountain Empire Older Citizens Inc. in Big Stone Gap, VA seeks a full-time PACE PCA Floater to assist with ADLs and IADLs, provide health-related activities, and help maintain the participant's home environment.

The role includes travel to participant homes as needed and center-based support. The position requires a valid driver’s license, ability to perform duties, and 1+ year experience with frail elderly, plus completion of PCA/Nurse’s Aide curriculum, CPR/First Aid, and immunization

Qualifications

• Must possess a valid driver’s license.
• Physical ability to perform assigned duties.
• One or more years experience with the frail elderly.
• Completion of approved curriculum as a personal care aide, nurse’s aide or geriatric nursing assistant.
• Annual completion of eight hours of continuing education.
• CPR/First Aid training.
• Medically cleared for communicable diseases with up-to-date immunizations before participant contact.

Responsibilities

• Provide direct assistance to participants – in the home or at the PACE Center – for ADLs and IADLs.

• Assist in maintaining cleanliness in the center and home environments including housekeeping tasks.
• Check temperature, pulse, respiration, blood pressure and weights and report as required.

• Respond to emergencies and ensure safety for participants.

Salary Range: $31,200.00 To $34,320.00 Annually

JOB SUMMARY: Performs patient care activities and other tasks as assigned to assist the primary care, therapy, and adult day health care staff. Additionally, this position involves the maintenance of an impaired individual in his/her own home.

JOB RESPONSIBILITIES:

• Provide direct assistance to participants – in the home or at the PACE Center – for the performance of basic Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
• Provide health and other therapeutic related activities as prescribed in plans of care.
• Assist in maintenance of cleanliness and neatness in the center and home environments including housekeeping chores.
• Assist in checking temperature, pulse, respiration, blood pressure and weights and recording and reporting as required.
• Demonstrate infection control techniques by using Standard Precautions.
• Demonstrate concern for all participants’ welfare by rendering immediate assistance to any participant in need.
• Show sensitivity to individual needs, be aware of each individual’s abilities, and help individuals maintain self-worth and dignity.
• Monitor participants for safety and wellness and keep the Center Manager advised of changes in participants’ condition.
• Respond appropriately to emergency situations.
• Report any suspected abuse, neglect, or exploitation to the Center Manager.
• Exhibit complete confidentiality at all times in all matters in regard to participants, their families, and other staff of the PACE Center by observing all relevant Federal and state laws, rules and regulations and all ethical rules of conduct.
• Maintain daily logs and progress notes as prescribed by Center Manager or designee.
• Participate in the Quality Improvement program.
• Perform other appropriate duties as directed by the Director, Center Manager (or designee) including, but not limited to the traveling to participant homes to assist with home care.
