Centra Medical Group is seeking an Advanced Practice Provider to join Centra’s PACE program in Lynchburg, VA. The role focuses on delivering comprehensive, person-centered care across in-home, community, and facility settings within a capitated care model.

The position emphasizes collaboration within an interdisciplinary team, with a focus on Elderly patient care and continuity of relationships, under ThriveAP guidance.

Qualifications

• Graduate of an accredited Nursing Program.
• Masters of Science Degree in Nursing (MSN).
• 2 years of Nurse Practitioner experience.
• Current Virginia State Board of Nursing licensure as a Nurse Practitioner with prescriptive authority; maintain BLS and ACLS certifications.

Responsibilities

• Monday–Friday 8:00am–5:00pm with no weekends or holidays.
• Collaborate with interdisciplinary team to deliver patient-centered care.
• Manage 5 patients per day with small volume.
• On-call every 7 weeks 5pm–8am; triaged before escalation.
• Build meaningful relationships with elderly patients.
