Job summary

PACE Partners of Northeast Florida seeks an Intake & Outreach Coordinator to drive participant growth by developing referral sources, conducting community outreach, and enrolling 3–5 new participants monthly. Qualifications

  • Bachelor’s degree preferred.

  • Minimum 2 years of documented sales success, preferably in medical services.
  • Clinical background preferred.

PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.


Position Summary

Under the direction of the PACE Executive Director and the Outreach & Enrollment Manager, the Intake & Outreach Coordinator is responsible for supporting participant growth through referral source development, community outreach, generating qualified referrals from assigned accounts/territory, and enrolling (net) 3-5 new PACE participants each month. This role serves as a primary liaison to assigned clinical and community referral partners and supports the intake and enrollment process for prospective PACE participants.


In addition, the Coordinator conducts compliant educational outreach regarding the PACE program model, eligibility criteria, benefits, and enrollment process; facilitates timely referral intake and follow-up; and collaborates closely with Marketing & Enrollment Manager and the interdisciplinary team (IDT) to support efficient and appropriate enrollment. All activities are performed in accordance with CMS PACE marketing guidelines, state regulations, and organizational policies.


Essential Functions

Referral Source Development and Pipeline Ownership


  • Maintain assigned referral source relationships across clinical and community settings, including primary care practices, hospitals, skilled nursing facilities, assisted living communities, Area Agencies on Aging, senior centers, senior housing communities, and other organizations serving older adults.

  • Serve as the primary day-to-day contact for assigned referral sources, ensuring consistent communication, follow-up, and education regarding PACE services.

  • Actively manage an assigned referral pipeline, including prospecting new referral relationships, nurturing existing partners, and re-engaging inactive referral sources.

  • Conduct compliant educational outreach to referral partners regarding the PACE program model, eligibility criteria, enrollment process, and participant benefits.

  • Track referral activity, pipeline status, and outcomes for assigned accounts and communicate updates to the Outreach and Enrollment Manager.

  • Identify opportunities to deepen referral relationships and improve referral quality and volume and elevate barriers or challenges to the Manager for support.


Community Outreach and Relationship Engagement


  • Participate in community outreach events, educational sessions, and presentations as assigned to support referral development and program awareness.

  • Represent PACE Partners professionally and consistently within the community and service area.

  • Maintain regular visibility with assigned referral sources through scheduled visits, follow-up communications, and educational touchpoints.

  • Support collaboration between referral partners and internal clinical and enrollment teams to facilitate smooth transitions into the PACE program.


Coordinate referral intake for assigned pipeline accounts, ensuring accurate collection of preliminary information and timely handoff to Medicaid Specialists.


  • Support eligibility screening, assessment scheduling, and enrollment activities under the direction of the Marketing and Enrollment Manager.

  • Maintain proactive follow-up with prospective participants and families to support timely progression through the enrollment process.

  • Communicate enrollment status updates to referral sources and internal teams as directed.

  • Identify and elevate enrollment delays or barriers impacting assigned referrals.


Enrollment Tracking and Documentation


  • Enter and maintain accurate referral, intake, and enrollment data within the CRM and other tracking systems.

  • Monitor assigned referral pipeline activity, conversion progress, and enrollment outcomes.

  • Provide routine updates on referral activity and pipeline status to the Outreach and Enrollment Manager.

  • Support data integrity, documentation standards, and reporting requirements related to outreach and enrollment.


Educational Materials and Program Representation


  • Utilize only approved educational and outreach materials when engaging referral sources, community organizations, and families.

  • Support the distribution and tracking of approved marketing and educational materials.

  • Ensure all information shared externally is accurate, consistent, and aligned with organizational policies and regulatory requirements.


Compliance and Regulatory Adherence


  • Perform all outreach, referral development, intake, and enrollment activities in compliance with CMS PACE marketing guidelines, state regulations, and organizational policies.

  • Maintain a clear distinction between education and marketing activities and avoid prohibited inducements.

  • Identify and report complaints, concerns, or grievances encountered during outreach or intake activities in accordance with established procedures.

  • Complete required compliance, enrollment, and participant-rights training.


Team Collaboration and Administrative Support


  • Collaborate closely with the Marketing and Enrollment Manager, Lead Community Outreach Coordinator, Medicaid Specialists, and interdisciplinary team members to support enrollment operations.

  • Participate in IDT or enrollment-related meetings as assigned.

  • Maintain flexibility in work hours to support outreach events and enrollment needs.

  • Demonstrate punctuality, professionalism, and regular attendance.

  • Perform other duties as assigned.


Qualifications

Knowledge and Critical Skills


  • Fluent English, secondary language a benefit

  • Experience working with a Medicare/Medicaid waiver program or demonstration project preferred.

  • Demonstrates experience in human service care delivery system

  • Experience with service providers in a community-based setting, acute or long-term care facilities preferred

  • Experience and knowledge of social service principles and practices relating to elderly population

  • Working knowledge of physical, psychosocial, behavioral, and family needs of the elderly

  • Knowledge of the Medicare and Medicaid programs

  • Capability to relate this information to determine eligibility requirements for PACE Partners

  • Working knowledge of service area community health and social service delivery systems and aging networks

  • Effective written and oral communication skills

  • Strong interpersonal skills

  • Ability to effectively present program model and philosophy to individuals, groups, and families

  • Strong communication skills with ability to listen

  • Ability to communicate and articulate PACE Partners services and benefits to medical and senior services professionals in a clear and concise manner

  • Excellent presentation skills for both small and large audiences

  • Ability to work autonomously yet support the needs of PACE Partners and your fellow team members


Experience


  • Minimum three years’ experience within marketing, public relations, business development, outreach, or social services type activities

  • Minimum two years of documented sales success, preferably in selling services to medical professionals within the Jacksonville area

  • Clinical background preferred

  • Have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.


License / Certification


  • First Aid and CPR certification is important, but not mandatory

  • Current State of Florida driver’s license with driving record acceptable to PACE Partners insurance carrier, and reliable transportation Customer Relationship Management (CRM) and sales force automation systems experience required


Education


  • Bachelor’s degree in Marketing, Social Services, or other health related fields is preferred.


Other Requirements


  • Must be medically cleared of communicable diseases and up to date with immunizations

  • Computer proficiency with Microsoft Office (Word, Excel, PowerPoint, and Outlook)
