Job summary

Chapters Health System in Florida seeks a dedicated CNA to provide compassionate patient care under the supervision of the primary nurse.

You will perform hands-on assistance with daily activities, ensure safety, and document observations while collaborating with the care team to deliver high-quality service. A valid Florida CNA license, CPR, and BLS are required.

Qualifications

  • High school diploma or GED is required.
  • Completion of a qualified Certified Nurse Assistant (CNA) training program.
  • Active Florida CNA license is required.
  • Must successfully complete a competency evaluation program before providing services.
  • CPR certification is required.
  • Valid Florida driver’s license with good record and automobile insurance.
  • Clear Level 2 background check.
  • Active BLS certification from AHA or Red Cross.
  • Alzheimer’s Disease and Related Dementias training completion is required.

Responsibilities

  • Adhere to written and oral instructions from the primary nurse.
  • Perform all aspects of personal care and hygiene per policy.
  • Use safe techniques for ambulation, transfers, positioning and ROM; follow standard precautions.
  • Guide families on safe, comfortable patient care including ADLs and mobility.
  • Assist families of patients as needed.
  • Document care and services, observations and interventions to the primary nurse, managers and IDG.

Responsibilities
  • Adhere to written and oral instructions prepared by the primary nurse.
  • Perform all aspects of personal care and hygiene to patients according to Hope Hospice Policy and Procedures.
  • Utilize knowledge of proper and safe techniques, re: ambulation, transfer, positioning, and ROM, Standard precautions.
  • Guide caregiver and families regarding the safe and comfortable personal care of the patient inclusive of but not limited to ADL’s, Standard Precautions, mobility and environment.
  • Assist families of patients as needed.
  • Document care and services, observations and interventions to primary nurse, managers and IDG.
Knowledge & Skills
  • Excellent communication; via phone, in person and email.
  • Good time management skills and organizational skills.
  • Attention to detail & ability to handle confidential information following HIPAA guidelines.
  • Knowledge of meal preparation and feeding techniques and/or questioning.
  • Must be flexible to work in other locations if needed.
Required Qualifications
  • High school diploma or general education degree (GED).
  • Certificate of completion of a qualified Certified Nurse Assistant training program.
  • Current Florida Certified Nursing License.
  • Must also successfully complete a competency evaluation program prior to furnishing services to patients.
  • CPR certification required.
  • Valid Florida Drivers’ License with good record.
  • Automobile Insurance.
  • Clear level 2 background check.
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Compensation Pay Range: $16.96 - $25.24.

This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug-Free Workplace Policy.
