Job Summary
Job Summary:
Performs various duties assisting in the treatment and care of participants in accordance with the established methods and procedures of the facility and in accordance with the participants plan of care.
Duties/Responsibilities:
- Perform personal care procedures as appropriate and indicated by the plan of care.
- Supervise and engage participants in the day/activity rooms while in center.
- Assist participants with ADLs such as toileting and eating, as needed.
- Participate in IDT as needed, including assessing participants and communicating plan of care to other Personal Care Attendants and Nurses.
- Assist participant in the transfers while assuring comfort. Provide personal care, ADLs, and general care routines in accordance with procedures to assure participants physical comfort.
- Take and records vital signs, height, weight (and other measurements, as applicable) in an accurate and precise manner. Prepare participants for meals, assists in feeding participant, if necessary, and distributes water and other nourishments according to participants needs and care plan.
- Makes rounds willingly, anticipates participant needs, and responds to participant needs promptly.
- Assume responsibility for participants clothing and personal property (prosthetic devices, hearing aids, glasses, etc.), securing items in accordance with established procedure.
- Assist with or transports participants, as necessary. Collect specimens, labels properly, and transports to proper area in a timely manner.
- Report and/or records any changes observed in condition or behavior to IDT.
- Assist in maintaining a safe and clean environment with well-stocked supplies and functioning equipment.
- Familiar with duties and performs them accurately, recognizing their importance in relationship to participant care.
- Follow care plan, observes established policy and procedures to assure proper treatment and care of participant. Report any observed change in participant condition or behavior to IDT
- Other duties as assigned.
Education, Licenses, Certifications and Experience:
- Current Long Term Care Aide Certification
- Home Health Aide Certification preferred
- Either one year working with the frail and elderly population or approved training
- CPR Certification
- High school diploma or equivalent.
