Job Details Job Location: San Jose, CA Salary Range: $31.00 Hourly

SUMMARY OF POSITION:

Under the direction and supervision of the Home Care Supervisor, the PACE Home Caregiver provides personal care, light housekeeping, and/or supervision to North East Medical Services PACE participants in the participant’s home.

ESSENTIAL JOB FUNCTIONS:

  • Provides assistance in the participant’s home according to the participant’s individualized care plan, and in coordination with the PACE Interdisciplinary Team (IDT), the participant, and the participant’s family. This may include assistance with:
    • Personal care including tasks such as peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
    • Housekeeping including tasks such as changing linens, laundry, and light cleaning.
  • Provides supervision and emotional support as needed for participants to perform necessary tasks.
  • Reports observed changes in participants’ conditions to the Home Care Supervisor and/or other PACE clinical staff.
  • Observes, tracks and reports participant status, including information such as vital signs and nutritional intake.
  • Proactively communicates participant health and safety concerns to the Home Care Supervisor and/or other program staff.
  • Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
  • Interacts in a friendly, courteous, and professional manner with participants and their families/other caregivers.
  • Participates in social and recreation activities with participants as appropriate.
  • Utilizes Epic appropriately and efficiently and serves as content and workflow expert for resolving issues and proposing solutions in Epic.
  • Performs other job duties as required by manager/supervisor.

QUALIFICATIONS:

Required:

  • Must complete and pass the North East Medical Services PACE training curriculum upon hire.
  • Current BLS and First Aid required.
  • Ability to communicate effectively.

Preferred:

  • Minimum 1 year of experience as a Personal Care Aide or EMT, or certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
  • Knowledge of the geriatric population.
  • Knowledge of safe and sanitary personal care techniques for the elderly population with physical limitations.
  • Knowledge of proper body mechanics during the provision of assistance with Activities of Daily Living.
  • Home Health Aide certification.
  • Certified Nursing Assistant (CNA) certification.
  • 1-year experience working with the frail or elderly population.

LANGUAGE:

  • Must be able to fluently speak, read, and write English.
  • Fluency in Chinese (Cantonese) is required.
  • Fluency in other languages is an asset.

STATUS:

This is an FLSA Non-exempt position.

This is not an OSHA high-risk position.

This is a full-time position.
