Position: Registered Nurse - Long Term Care

Shift Type/Shift Times/Hours/Day: Days, 6:00 AM - 2:30 AM, 8

Shift Notes: Days Shift 40hr gt

This position reports to the Director of In-Home Services or designee/Registered Nurse

Seeking a dedicated, experienced, and organized Registered Nurse to join our team.

  • This is a full-time position, Monday-Friday, with on-call weekends.
  • Candidate must be able to work autonomously and as an integral part of the Interdisciplinary Team.
  • In this position you will manage a caseload of participants in the Bucks and Montgomery County areas, providing skilled nursing care either in the participant's homes or in our health center.
  • In addition, the RN will manage and coordinate participant care needs with the IDT, develop and implement individualized care plans, conduct regulatory assessments, and provide ongoing care management to ensure the best possible outcomes.
  • This position does not require Oasis documentation.

Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute, and long-term care services so that older individuals can continue living in the community.

PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today's fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers, and policy makers. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Submission Requirements
  • 2-3 years full-time experience in an LTC facility with a geriatric population required
  • Minimum of one (1) year of documented experience working with a frail or elderly population, or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • BSN Preferred
  • Traveler must have their own vehicle for this need: Valid driver's license in good standing and proof of insurance required.
