PA - Registered Nurse- Long Term Care, Days+Calls+Wknds(8-hour shifts),**Must Receive Training from PACE Organization** - Feasterville-Trevose

Mobile Health Team Inc, a nurse-owned staffing agency, is seeking experienced, compassionate healthcare professionals for various placement options, including PRN, contract, and permanent roles. We are currently hiring for the position of Registered Nurse - Long Term Care in Feasterville-Trevose, PA.

Position Details
  • Start Date: 6/2/2025
  • Duration: 13 weeks
  • Location: Feasterville-Trevose, PA, 19053
  • Shift: Days, 6:00 AM - 2:30 PM, 8-hour shifts
  • Weekend Requirements: TBD/Holidays as needed
  • On-Call: Yes
Responsibilities
  • Manage caseloads of participants in Bucks and Montgomery County, providing skilled nursing care either at home or in our health center.
  • Coordinate care needs with the Interdisciplinary Team (IDT), develop and implement care plans, conduct assessments, and provide ongoing care management.
  • Work autonomously and as part of a team, ensuring high-quality care.
Qualifications
  • 2-3 years full-time experience in an LTC facility with a geriatric population.
  • At least 1 year of documented experience working with the elderly or frail populations; if less, must undergo training from PACE upon hiring.
  • BSN preferred.
  • Valid driver’s license and proof of insurance; own vehicle required.
  • State License: Pennsylvania (must be in hand at profile upload).
Additional Requirements
  • Work history, skills checklist current within 1 year, supervisory reference from past two years, license/certifications uploaded.
