Elaya Health is now hiring a Nurse Practitioner or Physician Assistant specialized in Endocrinology to provide on-site and telehealth care in skilled nursing facilities. The provider will evaluate patients, manage diabetes and other endocrine conditions, and collaborate with facility staff. This position offers competitive compensation, flexible scheduling, and full support in a mission-driven environment. Ideal candidates are licensed in NY or NJ and possess strong communication skills.

Qualifications

• Prior experience in skilled nursing, internal medicine, or endocrinology preferred.
• Willingness to obtain licensing in NY or NJ if not already licensed.

Nurse Practitioner or Physician Assistant – Endocrinology (Skilled Nursing Facilities)

Elaya Health, a rapidly growing endocrinology group dedicated to improving care in skilled nursing and long-term care facilities, is seeking a compassionate, highly skilled Nurse Practitioner or Physician Assistant to join our expanding team.

About the Role

We provide on‑site and telehealth endocrine care to residents in SNFs across NY, NJ, and Florida.

Our providers play a key role in improving outcomes for patients with diabetes and other endocrine disorders through proactive management, close collaboration with facility staff, and thoughtful communication with nursing and administration.

Responsibilities

• Perform on‑site patient evaluations and follow‑ups
• Manage diabetes, thyroid disorders, and other common endocrine conditions
• Collaborate with DONs, facility leadership, and primary teams
• Document in facility EMRs (training provided)
• Support initiatives that reduce unnecessary hospitalizations and improve A1C and finger‑stick outcomes

Requirements

• Licensed NP or PA in NY or NJ (or willingness to obtain)
