Elaya Health in Dayton, OH seeks a Nurse Practitioner or Physician Assistant to provide on-site endocrine care for residents in skilled nursing and long-term care facilities. You will manage diabetes, thyroid disease, and other endocrine conditions with support from a clinical team and administration.

The role offers substantial clinical autonomy, flexible scheduling, and comprehensive administrative backing, with compensation to $150k–$220k annually.

Formación

• Licensed Nurse Practitioner or Physician Assistant in New York and/or New Jersey, or willingness to obtain licensure.
• Experience in endocrinology, internal medicine, geriatrics, or skilled nursing preferred.
• Endocrinology experience is a plus but not required for the right candidate.
• Strong clinical judgment and communication skills.
• Comfortable working independently while collaborating with a larger clinical team.
• Patient-centered and committed to providing high-quality care to the long-term care population.

Responsabilidades

• Review blood glucose trends, laboratory results, and medication regimens
• Optimize diabetes management and help reduce unnecessary finger sticks and medication burden
• Collaborate with nursing staff, DONs, facility leadership, and primary care teams
• Provide clear clinical recommendations and ongoing follow-up
• Document in facility EMRs, with training and administrative support provided
• Help reduce avoidable hospitalizations through proactive specialty care

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout Dayton.

You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while working closely with facility nursing teams, primary care providers, and administration.
