Job summary

Mercy Haven, Inc. seeks a Full Time Residential Counselor for its Bay Shore Community Residence. You will support adults with severe mental illness, teach independent living skills, provide counseling and crisis management, and help with daily activities while documenting progress.

Responsibilities include plan monitoring, medication oversight, transportation, and advocacy to access services for discharge planning. Bay Shore location offers full benefits and PTO.

Qualifications

• High School Diploma or equivalent required; related experience preferred.
• Bachelor's Degree in Human Services preferred.
• Experience with people living with mental illness or other disabilities is highly preferred.

Responsibilities

• Participate in planning/evaluation of individual service plans and monitor plan objectives.
• Provide hands-on training and assist residents with activities of daily living and documentation.
• Accompany residents on recreational outings and travel training exercises.
• Provide counseling, behavioral intervention, and crisis management.
• Assist in day-to-day management of the community residence and maintain residence.
• Monitor medications and ensure safety, including incident reporting as needed.
• Advocate for residents' benefits and connect them with local services for discharge planning.
• Write progress notes daily and coordinate with staff and service providers.
