Community Health Resources, Inc. is seeking a Residential Aide for the Milestone program in Putnam, CT. The role supports residents with daily living, medication monitoring, and skill-building activities during the night shift (Tuesday–Saturday, 12am–8:30am).

Formación

• Education: High School Diploma or equivalent.
• Experience: Experience in Human Services with responsible work history.
• Licensure/Certification: Valid drivers license required with good driving record; CPR & First Aid Certification required or within 90 days of employment.

Responsabilidades

• Maintains physical environment and program standards per CHR, DMHAS, JCAHO and licensing standards.
• Monitors resident medications per policy.
• Uses effective communication to transfer information to staff about days events.
• Teaches residents rehab areas: nutrition, hygiene, budgeting, home care skills.
• Assists/Manages administrative tasks as directed by supervisor.
• Prepares and facilitates educational and skill-building groups and interventions.
• Collaborates with providers and families for coordinated care.
