Job summary

Civita Health in New Hampshire is seeking an experienced RN or LPN for overnight shifts, 24 or 32 hours per week, including every other weekend. You will monitor residents, perform assessments, and communicate changes promptly to ensure safe, consistent care.

You will document care in the EHR, adhere to HIPAA, and collaborate with a multidisciplinary team to provide timely, high-quality nursing care during night hours.

Qualifications

• Must hold an RN or LPN license applicable to the role and work location.
• Nursing experience with senior care, long-term care, or memory care is relevant.
• Knowledge of medication administration, vital signs, infection control, and clinical documentation; familiarity with EHR is helpful.
• Clear communication and dependable overnight attendance; available for 24 or 32 hours per week including every other weekend.
• BLS/CPR training is preferred.

Duties

• Assess residents, monitor vital signs, and respond to changes in condition, pain, or other care needs.

• Administer medications and provide nursing care within your license and established care plans.

• Support catheter care, tube feeding, and other assigned clinical needs; follow infection control and safety practices.

• Document care accurately in electronic health records (EHR) and maintain confidentiality in accordance with HIPAA.

• Communicate with residents, caregivers, and the care team, including during shift handoffs and urgent situations.

Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license applicable to the role and work location.

• Nursing experience and sound patient assessment skills; senior care, long-term care, skilled nursing, assisted living, or memory care experience is relevant.

• Knowledge of medication administration, vital signs, infection control, and clinical documentation; familiarity with EHR systems is helpful.

• Clear communication, dependable overnight attendance, and availability for 24 or 32 hours per week, including every other weekend.

• Basic life support (BLS) or cardiopulmonary resuscitation (CPR) training is preferred.
