Qualifications

• Licensed Practical Nurse (LPN) in good standing with state licensing agency.
• CPR/AED certification required.
• Minimum of one year as an LPN in a senior care setting preferred.
• Experience with computerized resident documentation systems preferred.
• Good understanding of senior needs and dementia care preferred.
• Ability to work with others and maintain confidentiality under pressure.
• Must be at least 18 years old; state-specific guidelines may apply.
• Valid driver’s license and acceptable MV?R if driving responsibilities exist.

Responsibilities

• Provide daily supervision and guidance to assigned non-licensed team members.
• Monitor resident wellness, identify changes in condition, respond to medication needs.
• Administer prescribed medications and support continuity of care.
• Make work assignments and help identify opportunities to improve resident care.
• Communicate with residents, families, physicians, pharmacies, and providers to coordinate care.
• Provide first aid as required and complete duties per company procedures.
• Participate in meetings and Relias training as required.
• Ensure compliance with residents’ rights and report violations.
