Formación

• Current Pennsylvania LPN license
• BLS/CPR certification required
• Homecare, pediatric, or chronic care experience preferred
• Ability to work independently in a homecare setting

Responsabilidades

• Provide skilled nursing care in the home setting
• Monitor patient condition and communicate changes to the clinical team
• Administer medications and treatments as prescribed
• Assist with activities of daily living and personal care
• Perform skilled nursing interventions including wound care and medication management
• Educate patients and families on care plans
• Maintain accurate and timely documentation
• Collaborate with physicians and care teams for quality outcomes
